Really like this SP, your best one yet (for me).

I like the human interjection into the piece, just the right amount I thought.

I thought a play around with line breaks may enhance slightly.

he comes back from afghanistan
paralysed from the neck down
she has him ensconced in the attic
so he can see the sky
through the skylight window
and fetches a new man home most nights
to fuck her in the bedroom below

voicebox gone he blinks
once for no twice for yes
she learns his fears
learns ...

your going to fill me up
you say
with sperm
the way a confectioner
fills a donut
with cream

have me curl up
tightly
for insertion

from my window
autumn birch trees
run like a tiger
in the wind
